Wish I had made my comments before reading all the others'. This is a very emotional image, portraying a truly empty promise. Alastair summed it up perfectly.
I think AA should put this image on their literature.

A very strong message/image. Portrays the title very well and would be a good one for a poster - re acholol abuse. My only slight comment would be that maybe the girl/backgroud is too well lit. Maybe duller/greyer or something would makes more impact of how awful it is to be left with broken promises, again. But I think the strong focus and calrity on the bottle really bits home the message

Friends, let me tell u about the broken promise. Judi and that lady there had a promise that they'll share that drink. But the day on which Judi created the angel picture, she was badly in need of a drink. So, she opened the cellar and guzzled off the bottle at one go... gulp, gulp, gulp. The next day this lady came in and found that out. Now, u can see her sobbing with her head buried in her hands. :)
Jokes apart, great composition and toning. Like it very much. Once again a strong visual with a softer emotional touch. Cheers
